If you are a patient, the easiest way to schedule an appointment is to call our office. If your referring physician is scheduling, there are three ways to schedule the appointment:

Print out a copy of the Scheduling Form (here), fill in the information, and Fax it to Choice Imaging at (260) 423-1421

For Electronic Scheduling please fill out the form by clicking on the button below, and we will call and schedule patient for you (if not already scheduled). We will let you know ASAP if a pre-cert is required. Otherwise, if you have already obtained a pre-cert then please list in the appropriate form box.
